How to refer — 4 steps

  1. Submit the referral — call (978) 722-4325, fax (978) 633-3130, or use the online referral form. (The 978 area code is HearthWell’s confirmed line, not an error.)
  2. Insurance verification — we verify Anthem BCBS benefits and report coverage + any out-of-pocket cost back to you before intake, typically within 24 hours. Private-pay rates are also available.
  3. Intake scheduled — a confidential assessment, typically within 48 hours, with a level-of-care recommendation.
  4. Ongoing communication — with a signed release (ROI): intake confirmation, progress updates, and coordinated discharge/step-down planning with a warm handoff to the next level of care.

What to include with your referral

  • Client name, date of birth, and contact information
  • A brief clinical summary (diagnosis, substances used, treatment history)
  • Current medications — especially any MAT (medication-assisted treatment)
  • Insurance information (Anthem BCBS) or private-pay intent
  • Level-of-care recommendation (PHP vs. IOP), or ask us to assess
  • Any safety concerns or special needs

How quickly can a referred client be seen?
Benefits are typically verified within 24 hours and intake is typically scheduled within 48 hours of verification. If your client needs care faster than outpatient admission allows, direct them to 911 or 988 and send us the referral for follow-up.

Will I get updates after the handoff?
Yes — with a signed release of information. You’ll receive confirmation that intake occurred, periodic treatment updates, and transition planning when your client steps up, steps down, or completes the program. Tell us your preferred contact method on the referral.

What insurance do you accept?
HearthWell accepts Anthem Blue Cross Blue Shield. Private-pay rates are also available. Our admissions team verifies benefits over the phone, so an unclear coverage picture doesn’t need to delay your referral — send it and we’ll sort out benefits with the client.
