Partial Hospitalization Program (PHP)

A higher level of support without residential treatment.

PHP is our most structured outpatient level of care — comprehensive, day-based treatment for when you need more than weekly therapy but don’t require 24-hour care. It’s often a strong fit when you’re building early stability or stepping down from a hospital or detox setting.

IOP is designed for people who are ready for more independence while continuing structured clinical support. Whether you’re stepping down from a higher level of care or beginning treatment here, it helps you strengthen your recovery while balancing work, school, or family.

Our Clinical Foundation

Structure only matters if the care inside it is sound. HearthWell’s programs are built on established, evidence-based clinical practice.

  • ASAM-aligned levels of care. Our PHP corresponds to ASAM Level 2.5 and our IOP to ASAM Level 2.1 — the nationally recognized framework for matching treatment intensity to clinical need.
  • Cognitive Behavioral Therapy (CBT). Practical work on the thought and behavior patterns that drive use.
  • Dialectical Behavior Therapy (DBT). Concrete skills for managing intense emotions and staying steady under stress.
  • Motivational Interviewing (MI). Counseling that works with your own reasons for change instead of lecturing you.
  • Relapse-prevention planning. Identifying your risk situations early and building a specific plan for them.
  • Trauma-informed care. Your treatment takes into account what you’ve lived through, not just the substance use itself.
  • Mental health and substance use treatment. Care addresses substance use alongside co-occurring mental health needs.
  • MAT coordination. For clients engaged in our programs, we coordinate directly with prescribers to support medication-assisted treatment (MAT); MAT requires an evaluation with the provider, and the client must be engaged in services at HearthWell.

Depending on what’s clinically relevant to you, ongoing tracking can include:

  • Attendance and engagement — showing up and taking part.
  • Cravings and triggers — what’s getting easier, and what’s still hard.
  • Mental health symptoms — such as depression, anxiety, or trauma responses.
  • Recovery capital — the relationships, stability, employment, and sense of purpose that support lasting recovery.
  • Drug screening when clinically indicated — objective accountability.
  • Emergency-services use — staying out of crisis.
  • Follow-up after discharge (FUA) — checking that you’re still engaged and supported at 30, 60, and 90 days.

PHP or IOP — which is right for you? You don’t have to answer that on your own. Every person who reaches out starts with a full, confidential assessment — a real conversation with our admissions team to understand your situation and find the level of care that fits your life right now. Here’s the general difference: PHP is the more structured level, with more treatment hours each week — often the right fit when you need more support, or you’re stepping down from a hospital or detox. IOP offers more independence with fewer weekly hours, so you can balance treatment with work, school, or family.

If you need a different kind of care first, we’ll tell you honestly and help you find it. Some people need medical support through withdrawal (detox), round-the-clock care, or emergency help before outpatient treatment is the right fit — HearthWell doesn’t provide those on-site, and if your assessment points there, you’ll get real referral guidance and a concrete next step, not a closed door.
